Re-Interview Request form must be filled out in detail (using descriptive words and details, in complete sentences), and can be returned to this office via email to miriah@swlcat.org or USPS. Along with the form you will need to provide 450 hours worth of paystubs to show hours worked in the trade since your previous interview date (must be 90 days since your interview date and hours must have been gained since that date). These forms will be held until the next determination meeting at which time you will receive word back from SWLCAT offices.
